Hypertension (High Pressure) Care

Advantage Care aims to help you reduce the risks of ill health associated with high blood pressure (also called hypertension). Genetic factors or lifestyle choices such as poor diet, being overweight, smoking, drinking excessive amounts of alcohol or suffering from diabetes all increase the risk of high blood pressure.

We know that even small lifestyle changes can make a real difference. We offer you the specialist care and support you need to make these lifestyle changes.

As one of the UK’s leading care providers, we can support you to:

• Maintain a healthy diet that is low in sodium, saturated fats and added sugar by planning healthy meals that you enjoy, helping you buy and prepare the right food and drink, or encouraging you to stay on track with your goals
• Include regular, achievable exercise to your daily routine
• Keep up social contacts and activities that are important to you
• Take your medicines at the right time and attend medical appointments when needed

We can provide general care and support for all aspects of daily living, including personal care and housework.

What does high blood pressure mean?

Nearly a third of adults in the UK suffer from high blood pressure. This disease is often a lifelong condition and increasingly affects children and teenagers too. It is sometimes called the ‘silent killer’ because people often do not realise they have it until their condition has become quite serious.

Blood pressure refers to the force of blood pressing against the walls of the arteries (large blood vessels) as the heart pumps it around the body. If your blood pressure is too high, this increases the heart’s workload and can damage the arteries. Early diagnosis is important, because left untreated high blood pressure can increase the risk of heart and kidney disease, stroke and brain disease.

Health and Wellbeing checks

30-minute regular visits to make sure your vitals are stable
Our Health and Wellbeing Checks service is the perfect way to start your care journey. It involves a 30-minute visit from one of our professional care champions to track your key vital signs. This can help support you by alerting you, a family member and your key healthcare professionals to any changes that could show that something isn’t quite right.
This unique service can spot early signs of a decline in a person’s physical, mental and emotional wellbeing. Getting help quickly could prevent further deterioration and the need to go to hospital.

The key vital signs that our Care Champions track include:
• Blood pressure and pulse
• Temperature
• Breathing and oxygen saturation
• Alertness
• General wellbeing which is assessed through a questionnaire

How can you or your loved one benefit from this new service?

• Results are recorded digitally using Bluetooth enabled equipment
• Results can instantly determine if any action needs to be taken using the NEWS2 matrix
• In certain circumstances, it could mean that a hospital admittance is avoided
• Advantage Care can quickly notify a family member or a health professional

How often does a Health and Wellbeing Check take place?

The visits can be as often as you need. They would usually take place once a week, every week at the same time.

We can provide the Health and Wellbeing Checks as a stand-alone service, part of a new tailor-made package of care or as an addition to an existing package of care.

What is the NEWS2 matrix?

The NEWS2 (National Early Warning Score) is a standardised matrix developed by the Royal College of Physicians which improves the detection and response to people at risk of deteriorating. To read more about the NEWS2 score please visit the NHS website.
